What causes this

The upstream provider rejected the call. The cause is on their side — a permission the customer did not grant, a malformed path, or an outage.

How to fix it

Read the provider's status from message. A 4xx means the provider rejected the call — verify the path against their API reference and the scopes granted at connect time. A 5xx is theirs to fix: retry with backoff.
